Ok I'm going to say this. If Phil is in his 30s, why the fuck is he sitting in a children's hospital??? Don't know what the age limit is there, but in LAX and Vegas, you get hurt and you're 21 years old, they take you to the children's hospital. Once you're 22, you get rushed to the big boy hospital. But hey, maybe Phil is secretly a pedophile who gets off to children with disabilities like tretcher-collins.

Dr. Dugi is not listed as provider under Pediatric Urology nor Sexual Development, most likely meaning he does not treat/operate on children. I don't know of any pediatric hospitals in the US that perform GRS on children (and certainly not on a 32 year old man).
 
His apartment in that photo... you know in thrillers when they go to the serial killer's home and it's like an insight into the sheer horror of the murderer's mind? It's like that, except instead of a string of hookers, he's killed his future prospects.

He has it in his tiny autistic mind that because he thinks cycling is cool and badass, so does everyone.

It’s also another instance of his tendency to try too hard with his lies. To him, wearing a cycle helmet proves that he totally cycles all the time, as opposed to making it look like his tard wrangler forced him to wear it because he keeps falling over.

I understand that when he was sleeping under bridges, he would bump his head on the bridge when he woke up. So in a sense, it is a tard helmet.

That was his bullshit excuse for wearing the cycling helmet all the time to look cool. Wonder if he still wears it around his apartment? He obviously does for dress-up photos.

I'd love to know what goes through his mind when he decides to play dress-up and take photos.

I think, as with so much Phil does, autism plays a part. There's a lot of social cues and subtleties that an autistic person doesn't get, and they tend to be focused on what we might call "rules." What Phil knows is that when a person dresses a certain way and poses a certain way, that means something. When he wears his ninja gear and poses with a gun, he thinks he looks exactly like the freedom fighters he faps over. His vacant expression, obesity and surroundings are irrelevant to him, because he is following the rules of what makes a person badass.

I think this is also why his lies are so hilariously bad. Take his belief that he passes as a woman - he knows a woman can have a shaved head and can wear fatigues and still look like a woman, so he sees no reason why he can't do the same, despite the fact that he looks like the big fat man he is. With the Latinx thing, he knows it is possible for a Latinx person to appear white, so again, he ignores the fact that he looks and acts like a honky and expects everyone to believe him when he speaks a few phrases of mispronounced Spanish. He's following what he imagines to be the rules of how a woman or Latina can appear, and can't get his head around the idea that other people can see through his lies.
