Even Crayon Shin-chan, a comedy show where the main character, a 5yo mischievous brat, interacts in over-the-top ways with family, friends and complete strangers, presented secondary transgendered characters in a positive light and that was in the 80-90s.
Yes, I am aware Crayon Shin-chan uses a lot of one-shot crazy trannies or transvestites as antagonists or for the lulz in movies (many of those end up defecting to the good guys' side, though), but there was this one recurring trans character in the actual series who didn't pass at all, yet was actually a genuinely nice person and appeared in several episodes where the moral was pretty much "don't judge people on appearances". Oh, and that character actually did things other than "just being trans/only cares about LGBT issues".
Actually, there seem to be a lot of trans and crossdressing characters in anime and Japanese videogames. They never milked that for tumblr "progressive points" though. Some play it for titillation, others for feels, but in general it tends to be just a side of a character instead of the whole character, at least the ones I've seen.
I just noticed Betsy's belt buckle is a female symbol. I guess she's a tumblrmisandristfeminist.

Doing some research, and it turns out that there is an animated series that has a trans lead, and it aired in 2011. The show is called Wandering Son, and the main protagonist is boy who wants to be a girl, and focuses on gender identity and puberty. More info here:

And, from where I learned about this show from (https://www.reddit.com/r/asktransge...n_zed_the_first_animated_series_to_feature_a/), Wandering Son is actually very well recived in transgender circles, while this series is reciving criticism from people in the same link.

Claudine...!, also by Ikeda, had a transman lead (though it never got an anime AFAIK).It's not quite as overtly about transgenderism as Wandering Son, but, if you go back to the 1970s, there's Rose of Versailles (a.k.a. Lady Oscar), which was trans-themed in a historical setting. (Here's the first episode of the anime version of Riyoko Ikeda's groudbreaking manga, uploaded legally to Youtube courtesy of Nozomi Entertainment.)
Rose of Versailles is one of the seminal and most influential shoujo manga and anime ever, and it's been adapted into stage musicals starring the crossdressing (female as male) Takarazuka troupe. It was also a heavy influence on Kunihiko Ikuhara's similarly genderfluid Revolutionary Girl Utena (which, from what I understand, in turn influenced Steven Universe in some ways).
