I think an inability to feel shame or embarrassment isn't really the symptom of any mental disorder, since those feelings are definitely instilled in you by family and society. A lot of wiggers and thots are getting away with murder these days via nebulous "behavioral disorders", like Oppositional Defiant Disorder, Generalized Anxiety, and even some personality disorders. These don't have anything to do with brain chemistry or trauma, just a bad upbringing, or shitty/weak personalities. The whole field of Cognitive Behavioral Therapy is one of the most effective in treating patients because these disorders can be reversed through conditioning. My completely layman and irrelevant opinion would be that Ralph probably has a couple behavioral disorders with one genuine low-grade mental disorder. I don't know much about his upbringing beyond his deaddad, though. I'd definitely be interested to learn wtf it was like.

Ralph had no choice but to own the fact that this happened to him. He couldn't just suddenly go radio silent in Portugal and reappear six months from now when the swelling/bruises finally go away and pretend nothing happened with new teeth and a stroked-out droop eye looking sideways. I'll give him props if he ever owns both how it happened and why it happened. Until that day comes this "chased down my four attackers" bullshit just don't compute.
