It's impossible that this surgeon declined because he's being asked to make miracles happen, no it must be solely because he's evil and hates trannies. The TiM in the comments links to the doctor's website. Dr. Adam Weinfeld, a plastic surgeon in Austin, Texas who went to Swarthmore for undergrad and Baylor for med school. He offers the full suite of normal plastic surgeries and has a section on his site for "gender affirming surgeries". He ostensibly does perform ffs according to his site—and his instagram has a post showing off one of his patient's ffs results as of 1 week ago—so it's actually a little unclear to me why he would turn down this specific TiM.

There had to have been some red flag that went undisclosed in the reddit post. My best guess comes from the "age based procedures" part of his site, where it states that there are issues with certain bone-related surgeries as people get older because their bones get more brittle and thus decreases what doctors can safely do. Perhaps this is an older TiM who has been taking anti-androgens and estrogen for years (he says in the OP that he's been transitioning for 9 years), which tends to diminish male bone density and health. Asking for a bunch of procedures that require fucking with his facial bones (nose, chin, jaw, and forehead) would then potentially be unsafe, opening up the doctor to liability. I don't know why the doctor wouldn't have just explained that if that were the case, but it's my best guess given that he does actively perform these surgeries on troons.
