It was interesting that he said he had "been infected". That could suggest something like hepatitis B or C. Jaundice isn't always present, even in late stage cirrhosis. Ammonia buildup leads to slurring, appearance of being drunk, extreme lethargy, sleep problems, irritability and personality changes.

Tarl reminds me of the dramatic goth chicks I knew in high school. Always had some bullshit they were on, having fits of rage followed by long crying sessions.
