I hate to be that person, but Striker hasn't just past the point of no return, but he's fucking flung himself off the precipice. He's too fat for surgery to help him and he's too unhealthy that any physical exertion is likely to do more harm than it would help. This isn't even beginning to touch on the fact that his mindset will probably refuse any sort of advice up to his inevitable deathbed.

That's the reality of it, really.
There's been cases where people who were even heavier than him have bounded back

But here's the difference, Striker doesn't seem like the kind of man to want to get skinny. Not when Null is out there.
I imagine Striker stress eating his way through a mountain of ice cream and Doritos as he cowers in his house, fearing Null's Black Panther assassins that are after him.
 
Then he's basically fucked since the only alternative to surgery would be to force him to burn all the fat, but since he's a sedimentary fuckwad the only thing that's ever been conceived in any form that would work is the Insanoflex from the Aqua Teen Hunger Force film...though given it would instantly replace body fat with overstimulated muscles it would only solve the wrong problem there.

More realistically, methinks they're going to force him into some kind of assisted living/physical therapy kind of deal where what he eats is strictly monitored and he's forced to do physical exercise. Oh, and no internet, shit's bad for him anyway.

I'm not an expert, but as I recall, there is a certain point where the body essentially realizes 'holy shit, I weigh far too much', and the amount of calories you have to consume to continue putting on weight increases almost exponentially. If the big bad wulf could decrease his caloric intake by even a few percent, he'd probably start dropping pounds surprisingly fast until he was back under a certain limit. This is why I refuse to believe that obesity is anything but the result of a lack of mental discipline*, since there are so many ways to lose weight with minimal effort out there, extremely fat people simply don't have anything else to blame.

* (excluding the cases where it can be irrefutably proven that a genuine eating disorder is responsible for the situation, which I acknowledge.)

He won't get better on his own, he really needs some kind of intervention

He seems to get off on pity or at least realizes it's the easiest kind of attention he can get. Hell, that can explain why he's into the whole soiling diapers thing. Like a few people have said, it either is a humiliation fetish or some really basic and disconnected way to get attention and he's not entirely aware why he does it. Considering he can barely commit to a full sentence I think it's the latter.

Hell, most people get that when they start having trouble moving. Striker needs a machine to help him breathe and he still hasn't gotten the memo.
 
I've never actually seen him make any negative comments about his weight before which is unusual considering how massive he is. Maybe he's either too stupid to realize how dangerous his situation is, or he's willingly refusing to improve because he's subconsciously hoping that his horrible health will eventually kill him.
 
Have we any concrete info on his parents? Just curious; I'm always interested in knowing how cases like this get into the situations they do.

Though instead of disowning him and sending Striker out onto the streets, they keep their precious 400lb+ high school dropout content with games and food in the back of their house, since his very existence is embarrassing to the family. He's indicated that his relationship with his brother is very strained as well, with said brother mostly trying to ignore Striker's very existence. Said brother is a fairly normal person, if a bit overweight, but he looks normal and has a life.

Between @DirkBloodStormKing indicating that Striker has had a past history of flashing women, and some other...Concerning things Striker has alluded to, it seems that some time ago, he might have done something particularly bad. Gotten into a lot of trouble, and at that point his life mostly fell apart. He dropped out of high school, became an ever inflating balloon in the back of his parent's house, and became the exceptionally creepy person we know today. Of course, this is all still speculation, but there has to have been something in particular that happened at some point to get him where he is, considering how normal everyone else seems to be.

Any indications that he pulled a Nick Bate? Aka tried getting a little sugar while it was still growing on the cane? It seems like it'd mesh fairly well with the degeneracy he perpetuates these days.
