I actually thought he had some kind of hallucinatory illness that made him become paranoid

I don't think you'll miss much. You won't get a few of the intended reactions, especially when TOS characters show up, but there's very few of those. References can work both ways and there's a huge historical gap between TOS and TNG, so it's no big deal. TNG is a long show though so you can always pace yourself with some TOS episodes in-between like I did, since they'e both super episodic.Star Trek noob here, I've only dipped my toe into the franchise but I've always found it interesting have been meaning to dive into it.

I would definitely want to watch TOS at some point, but TNG is what interests me the most since that's what I was first exposed to as a kid.

When watching TNG(to save yourself the time and avoid the significant amount of filler), I would get a fan roadmap(cliff notes if you will) which will make you watch only narratively critical or high quality episodes
Filler? TNG is an episodic television show. There is nothing there to "fill," you dunderhead.
Fan roadmaps are dumb because a dude who has watched every episode a dozen times is going to have a vastly different concept of what's "good" than someone watching it for the first time.
