It's obvious by these pics that at some point prior to the creation of the Killstream, Ralph lost a significant amount of weight and was left with those abominable sagging folds. He likely gained the weight back in an attempt to minimize them, but this didn't work, which is why the Gunt exists at all. This time, he was overweight AND had excess skin. No matter how much weight he gains or loses, the damage is done. The wage of overindulgence and gluttony is the Gunt, and it's the price he has to pay.

That being said, there is no reason he can't go for full body lift surgery in Mexico where it's significantly cheaper than the US. However this requires discipline on his part in preparation: saving pesos, abstaining from drugs, alcohol, and cigarettes. This would also require commitment to a goal, something he clearly has difficulty doing. This is something that he could improve his appearance and life, yet he chooses to spend his money on Mexican hookers and benzos.

It would be like what happened with Chris Chan in jail, everybody is amazed at how skinny and relatively normal-looking he is but then he makes a beeline to the shake shack as soon as he's out and within a month he's as fat as he's ever been.

Of course in Ralph's case he already has (literal) baggage in the form of loose skin so without significant cosmetic surgery he won't even have that "wow he almost looks normal!" phase that CWC had.

The gunt is like a reptile that sheds its skin as it grows, except in Ralph's case the old skin fails to detach so he's stuck dragging along the deflated remains of past gunts until he finally gets a mortal wound from crashing into piss bottles every time he passes out.

One interesting thought that occurred to me as I wrote this: just as an arborist estimates the age of a tree by counting its rings, the coroner who eventually autopsies Ralph will be able to confirm his age by counting the folds of his gunt.

You don’t even need to go to a chris Chan reference. Ralph did time for trying to punch a lady cop and came out the thinnest he’s been in his life (still had a huge head and midget body) and ballooned back up inside a year

I find it funnier because we've seen him pave this slope brick by brick only to predictably slip in his own filth and crash into slumber at new bottoms. Fucking himself over is the only thing he consistently puts effort into, his real work.

Do we cite Dante's Inferno and contrapasso too often here? Yes. Are there good people in bad situations? Yes. Is Ralph one of those? No. He is a shit-goblin sex pest in pursuit of buggery, violence, and debasement.
