The spade in this case simply refers to playing cards seen in the drawing. Despite this, I would not be surprised if it is a slight appeal to the BNWO group.

When searching the meaning of this phrase, the first result was a post on r/femboymemes asking about the meaning of the phrase. Users in the post's comments seemed to enjoy using the phrase. As was pointed out by Cheese_lad in this post, one of the commissions features a children's security buzzer. It is also worth noting that the same commission with the buzzer also features incest.
