It's self-deprecating humor. The joke is "I'm such an awful person that I would say something like this".

A lot of jokes are like this. Steve-O once did a stunt where he jerked off while parachuting, where the joke was "look how degenerate I am that I would film this and show you". It's self-deprecating humor.

I get that it's not particularly funny because a lot of people find the idea of forcibly transitioning someone extremely distasteful, but that is the joke, and the other person is laughing, and it seems like a huge assumption to say that they're uncomfortable.

I don't know anything about this guy so maybe I'm missing some context. I was just expecting some hard hitting, super gross thing about him being a pedo, and just got a livestream clip of him and his girlfriend/wife/whatever joking around.

This is probably the most accurate thing to say about the clip. I think it's an exaggeration to say he's admitting he's a pedo/whatever. But I do think treating the subject casually does indicate that the person is at the very least a porn addict and is wayyy too comfortable with joking about things like that.

Except that Dev has gotten to hot water before by associating himself with known pedophile Foodshops. When confronted about this, Dev said he would not end the association, since it "isn't directly happening in my server". This isn't his first controversy involving kids.
