Maybe there's a broken windows sort of thing going on there(assuming he's a meth head), they try to quit meth, roughs it out for a week or two then look in the mirror while sober and sees what he/she looks like. Why should he/she even try while looking like that? Their relatively sober self must feel absolute despair when they can look at themselves and what their lives are like.
 
I remember a time when people were tattooing themselves with all their favorite childhood characters, even though they were intended for small children. I'm sure that a lot of them have outgrown the idea by now.

The examples I have picked would be what people would consider the best possible outcome and probably photoshopped, some people would have been a lot less fortunate.
