It seems like she's been trying to drop 'subtle' (read: not subtle) hints about that for over a year now (while of course acting like she has no idea why her audience is mentioning ADHD). Okay, I just scrolled through @Extra Dainty Gorl 's breakdown, and Amber really needs to get her lies straight. She announced Outpatient to us in February 2022, saying she would start at the beginning of March. By end of March, her audience was suspecting that she quit. By April, she announced that she had quit, because they wouldn't help her lose weight and she needed to take weight loss seriously. Amber spoke of the WEEKS of therapy she received, and how she had learned so much about her BED. Amber hoped that she could keep working with her HAES psychologist for other, non-weight related issues she needed help with (such as anxiety, her past, etc). She talked about 'hoping' to work with her on and off until maybe May or so, where Amber announced that she could continue to work with her (but not part of outpatient). We hear almost nothing about the psychologist until late fall, when she tells us that the psychologist was going on maternity leave, and was unsure about proceeding with appointments because the new psychologist filling in with an icky, ICKY MAN! And then no talk of anything therapy related until late November/early December when Amber starts talking about getting WLS. And now she's 'quitting' the Outpatient program - she quit nearly 1 year ago.

These restrictions are on people over 40+ BMI for mine I should have clarified that

Medicaid has similar restrictions

• Over the age of 13 for a female and 15 for a male.
• Body Mass Index must be over 35 with at least one comorbidity.
  • Co-morbidities include sleep apnea, high blood pressure, high cholesterol, diabetes.
• If you are under 21, you must have a Body Mass Index (BMI) over 40 with at least one comorbidity.
• A letter from your primary care physician stating that weight loss surgery is medically necessary.
• Passes a psychological exam.
• Documentation showing that the patient tried to manage their comorbidities with standard treatment but they were not successful.
• The patient must complete and show documentation that he or she participated in a medically supervised weight loss program for 6 months and it happened within the last 12 months prior to surgery.
• The patient must understand they will be required to change their diet and lifestyle after surgery.
• Nutritional and psychological services must be available before and after surgery (usually from the physician’s office).

Bitch dont even have a PCP unless you count the ER. lol

That's not the Medicaid requirements in every state.

@Extra Dainty Gorl, a 6 hour long first appointment is definitely NOT normal. She had a long appointment the last time she went to a bariatric clinic because they did a pysch test on the day (362 or however many questions that are really just never/sometimes/always answers you aren't even supposed to think about but we know she poured over her answers) without actually having a pysch present, and she went with the cash already in hand so there was no reason to not do things quickly. The first appointment you don't even usually see the surgeon. It's usually just a GP or NP (that specialises in obesity) that runs through your general health history (mind you in Amber's case that alone could take 3 hours), and runs routine tests like BP, bloods, takes a starting weight, maybe O2 saturation etc. Someone will also talk to her about surgery requirements, and the clearances she'll need. Then she might see an admin person to discuss the cost and how she's going to pay for it if approved. They might have her see a dietician at this visit if there is one with a free appointment slot. Most of the clearances and appointments though don't happen on the first visit... they just get the referrals done so Amber can organise them herself. In her case she'll need referrals to psychologist, cardiologist, respiratory surgeon (including a sleep study), and dietician, at a minimum. If she gets through all those (she won't), then she'd likely also need clearance from the anesthecist.

The fact that she'll rock up with a journal full of inane questions, doesn't mean they'll waste their time pandering to her. The clinics I've experienced in my time, the first appointment is virtually scripted. No need to ask questions really because they cover pretty much everything in their script. The only real things up for discussion are how the patients individual health issues will affect things moving forward. In the US most practices have ongoing educational sessions as part of their monitored weightloss program required by insurers. Even if a patient is self pay, or their particular insurer doesn't require it, many surgeons will expect their patients to participate in that program while they get all of their clearances ticked off. Just getting those clearances and then booking the actual surgery can take months, so there's no need to rush everything into a single day.

In the olden days they stapled using one line of staples with a manual stapler. These days they use a pneumatic stapler that fires six lines of offset staples (two groups of three lines) with a cutter in the middle to ensure a minimum distance from cut to first line of staple. For those that watch MSHPL, this is why when Dr Now does a gastric sleeve surgery, he can fill the part of the stomach he removes to show the size better because the are three lines of offset staples sealing it.

While it's certainly possible to stretch the remnant stomach over time, over eating after bariatric surgery generally causes vomiting, and if it doesn't then the person will be in agony until the stomach volume is decreased by digestion. It's easy to sit back and think someone can massively eat beyond their pouch volume, but it's really not possible to eat through that level of pain. Weight gain after bariatric surgery comes from liquid calories, slider foods (like chips, cakes, icecream etc that dissolve pretty much before even hitting the stomach) and grazing on small quantities of food all day long.

OUTPATIENT UPDATE: Amber inadvertantly admits that she originally lied about outpatient, and only went for LESS THAN A WEEK. That means she quit outpatient BEFORE doing all of those 'mookbongs' in March that were supposed to be part of her outpatient therapy.

Amber says that she's been seeing her HAES psychologist for a year, and that's YET ANOTHER LIE. She saw her for less than one week in March, then didn't go back until May or so And then left around November or so, because the psychologist went on maternity leave and Amber didn't want therapy sessions with her male replacement. And even then, Amber said that she was seeing the psychologist SEPARATE from the outpatient program (because she rejected outpatient therapy to focus on calorie restriction and weight loss, which worked WONDERFULLY for her).

So, to recap: one week of HAES outpatient therapy, and about 6 months of crying about fatphobia and YouTube 'boolean', and calling it 'about a year of intensive outpatient therapy'.

Amber can't have an HAES therapist when trying to get approved for WLS, and she's going to miss the girl her psychologist. Amber's HAES specialist helped her see things from a new perspective - causing absolutely NO CHANGE in her mindset or behaviour.

WLS UPDATE: appointment officially scheduled. In a couple of weeks. No other details given. I wonder if this doctor will also tell her not to eat vegetables, but instead to eat beef jerky and McDonalds cheeseburgers without the bun? Amber's excited to go to the appointment to figure out the process - which was likely explained to her the last THREE times she went to the initial consultations. I'm sure this time will be different.

As I said, it really doesn't happen these days with the modern technology that a person can over eat their way to a burst stapleline. I'm talking here about when the stapleline bursts months to years after surgery. It didn't tend to be dangerous as such because in those days the stapleline wasn't completely across the stomach. Surgeons would leave a small opening, in the line for the food to pass through into the lower part of the stomach. No part of the stomach was removed or detached as it is now. Stapleline dehiscence during surgery is where life threatening leaks can happen. It's rare for first time bariatric patients, but much more common, (20x so) in revisional surgeries because of the difficulty in getting old scar tissue to seal together. This is why many surgeons are reluctant to perform revisions, and won't perform them on non-insurered patients. If a leak occurs during surgery, you're in open cheque book territory as hospital stays can run into many months with an unknown number of extra surgeries to try to correct. Surgeons generally will check for leaks before discharge, either by having the patient do a barium swallow test post surgery (that sucks with a new pouch and seems cruel) or it can be done during surgery by using a blue dye saline mix added to the pouch under pressure and examining the stapleline via the laprascopic camera to look for any leaks.

ETA: The ultralong appointment. I watched a short compilation and Amber talked about an "Intake Appointment". My guess is that these specific intake appointment days include a long group information session including talks from the surgeon, dietician, nurse, and on staff GP, their finance coordinator, with a Q&A afterwards. This session could be 3-4 hours and generally includes having baseline weight taken. They may then have individual appointments scheduled later in the day. She also talked about a long travel distance, so she's continuing with the out of state surgeon (a lie because she was asking about insurance covering surgery in Kentucky). She's talked about a celebrity surgeon, so that would tend to mean either Dr Smith at Kentucky Bariatric Institute, or Dr Proctor at Beltline Bariatics in Atlanta. The other "celebrity" bariatric surgeons are just too far away. I still believe she's angling for a TLC paid surgery.

Weight gain after bariatric surgery comes from liquid calories, slider foods (like chips, cakes, icecream etc that dissolve pretty much before even hitting the stomach) and grazing on small quantities of food all day long.

This will be Amber's (main) downfall if she actually got surgery. Over time the pouch expands slightly. That is not from the patient overeating but as part of the healing and body adjustment process. Even then, there are rules in place like not eating more than a cup of food at a time. The post WLS diet is mostly protein with low carb veggies (not keto since that is mainly fat, but still a relatively low carb diet). There are other recommendations to how the patient eats, but everything advised is intended to keep food in their smaller stomachs for longer. A new bariatric patient will not be hungry at first, but they usually begin to feel hunger within the first three to six months.

Going to how Amber would deal with slider foods specifically-

First she'll be one of those patients on MSHPL that cries to Dr Now that they have no hunger, what the fuck is wrong with her? Then she'll eat through a party sized bag of flaming hot Cheetos in one sitting no problem and realize she's figured out how to cheat. She will often force herself to eat massive amounts of food even when she's not hungry, too. She ate through those appetite suppressants, she ate through Ozempic. I don't think Amber would think much of making herself vomit or have a dumping episode after trying to eat a pint of Ben and Jerry's in one go. She'll feel uncomfortable, but when is she not in some sort of pain? She eats because not only is her life boring, shitty, and painful, but food also gives her pleasure in her boring, shitty, painful life.

She's hopeless. She's gonna die fat. Only question is how far into the future. She's no longer the 300 pound twentysomething that could've bounced back easier if she actually lost the weight. Deathfats start going down hard in their early thirties. Amber is 32.
