Rapid weight gain which causes severe stretch marks. Add gravity that causes the fat to spill over the belt line over time and causes those stretch marks to fold over one another and there it be. The gunt shape itself is cause by the naval passage struggling to maintain its shape.
 
Ralph's gunt is absolutely disgusting. The skin is all creased, grizzled, and lumpy... I've seen people older, fatter, and more decrepit than he is with smoother gunts than that. How did he even get to a point that it looks that bad? Consequences of all the Maker's Mark and xannies?
