These are not necessarily contradictory statements if the psychotherapist sets realistic expectations: for example he can (should?) tell the client that yes, you're a man and I'm sorry you're troubled by this fact. But we'll show you how, with proper attire, posture training, voice training, etc, you may become more womanly than you used to, perhaps to the point that the casual onlookers would be willing to look past some discrepancies. Realistic goals gives the clients a sense of achievement and serves to enhance the client's agency, which will improve his mental health. The point to tell the client not to ruminate on things that cannot be changed, but to work on things that can be.

What will not improve the client's mental health is to tell him he was indeed every inch a woman and that it is the world that is mistaken, for the therapist is thus pitching the client against the world, and please tell me how he is supposed to win!
