I've started transcribing this to text due to the legibility issues. I am up to page 4, and will get back to it later assuming nobody else finishes it or gets an AI to transcribe it. It should be faster later since I can copy the form questions and only transcribe the answers. I transcribed it into Notepad and only had two words I could not read; the illegibility of these words do not seriously impact the ability to read the sentences. I gave it one quick proofread. Hopefully I did not make any serious errors of transcription; any errors present in the text I have attempted to preserve.

The following pages are concerning what sounds like the oldest child. Based on the text, I believe this child is placed with Kayla's family.
Pages 5-8. I've switched to Notepad++ so I can use a darker theme due to eyestrain and to use multiple views for ease of copying/pasting/editing mostly repeated sections. Hopefully there are no errors remaining from my Replace All operations.

Page 8 was especially bad for legibility. Puzzling out some of the words in the second half of that page was a true challenge.

Despite all our jokes about Nick having a ton of free time now, his meetings with the case worker(s), any medical appointments the children have, and visitations means he still drives around for the children to an unknown degree. I also enjoyed a laugh at some of the safety goals and tasks that are definitely not being met.

Engagement in services is recommended immediately. Under MN Statute a parent must show significant progress in case planning for reunification by the time a child is 6 months out of the home, in which the timeline to achieve reunification could be extended up to 365 days from date of placement. If children are still out of home at 6 months, a permanency progress review hearing will be held to assess progress. Social worker will work with the family and meet at least monthly to assess progress.
