Conjecture about Kayla from what few facts are actually known along with statements made by Nick. This approaches the realm of fanfic, but I believe it is a logical scenario of what may have happened. None of this is meant to excuse or diagnose Kayla, merely speculate on what could have transpired in a similar situation.

Known Facts:
Kayla has had 8 pregnancies, 5 live births in nearly 20 years of marriage.
The 5 children are spaced roughly 2 years apart.
Kayla has expressed her feelings of inadequacy and failure as a mother and a woman.
There has been more than once (in my memory) in the last 6 years or so that Kayla has had some type of "procedure" requiring hospitalization and recovery.
Nick has stated that Kayla's body was "destroyed" by pregnancy.
Kayla seemed concerned that she get her prescription drugs while in jail.
Now my conjecture and fanfic.
The obvious mental issues that Kayla may have suffered are post-partum depression, compounded by grief over loss of 3 pregnancies. Woman who miscarry often feel inadequate and a sense of guilt and shame. There is also the physical toll pregnancy can take on the body, especially if the pregnancies are in rapid succession.
I suspect that Kayla had some physical complications from the many pregnancies, perhaps ending up with a prolapsed uterus, which would have made vaginal sex painful and may have resulted in surgical repair. I have wondered if when Nick talked about Kayla's "destroyed" body, he was not so much negging her, as much as he was repeating what Kayla said about her own body. The flabby, stretch marked abdomen and the windsock breasts don't do much for one's self esteem, especially if you know your partner does not find your body sexually attractive. That would explain the mommy makeover surgeries. She had become the hausfrau brood mare and not the cute wife of an important non-practicing lawyer. Her insecurities are compounded because all of a sudden, he has new friends that she doesn't know, and he stays up all night drinking and laughing with them, meanwhile she goes to bed alone, her partner having more fun being a YouToob influencer and sleeping off the hangover all day, instead of being a father and partner.
A recipe for marital disaster.

So multiple surgeries, probably corrective/hysterectomy and later, mommy makeovers.
Post-surgery pain meds.
Perhaps meds for chronic pain.
Prescription meds to treat both depression and pain.
So, Kayla takes to her bed and floats away on SSRI's and opiates and Star Trek.
This is the recipe for prescription drug addiction. I would not be surprised if Kayla was the first drug addict in this co-dependent relationship. What a contemptable, weak woman. She chose the selfish, easy way to escape when she had all the resources to get well. What a coward. What a failure.
The cocaine came later and was probably Nick's drug of choice. It gave him the energy and kept him awake so he could drink all night and take over what had been Kayla's job during the day.

In summary, this fanfic is a peek into what might have happened. The names have not been changed, to condemn the guilty. I think it possible that Kayla had physical pain and mental health issues, became a script drug addict, abdicated her participation in her children's lives and Nicks bed, thus Nick's "worst day". Instead of mustering all his resources in finding help and admit that Kayla was in trouble and their marriage was floundering, Nick the porn-sick egotist knew better and decided that he could fix her, fix everything, He didn't need anyone to help because after all, he is the smartest guy in the room.
A man so fragile, he could not ask for help, would not admit fault, and would rather watch his children suffer.
Or did he even notice their pain?
I bet that cocaine and MDMA made him feel like a real Superman.
What a weak, cowardly man.
What a failure.

I have no doubt Kayla gave him some affection, as lonely druggies are want to do, but it wasn't this secret, sensual romance as he portrays it. Kayla was bored, high, and he was willing. It seems pretty clear that while Nicky's "medicine" of choice was booze and later coke, Kayla's was actual prescription pills. Do we know what medicine it was that she had delivered to her in jail, and/or do we know if she is on benzos? Benzos such as klonopin can be prescribed for general anxiety/"muh mentals" type stuff and as our boy Jorden Peterson found out, they are no joke.

I would add something to this - when Nick started his youtoob thing, nobody knew how far is he gonna get. I wonder what Kayla thought about it at first. Her husband was meant to be this lawyer with a stable career, and as a good wife, she would continue to support him in building up his skills and experience as a reputable lawyer.
But then he got into some childish comics, and some guy called Vic Lasagna, and just having this retarded hobby where he talks to people and they send him pocket change. What sort of career is that?

And then he blew up, with Rittenhouse, with Depp. And I wonder if this did a number on Kayla too, that her nerd husband discovered this golden goose, that is seemingly easy to get - even this dork made a fortune off of it. But I can see how her achievements as a mother could've been completely overshadowed by Nick's unexpected success. And this could've messed with her mind too.

I wish someone could find this because in other versions its “ could have difficulties having kids” and I’ve heard tumors and cysts in various posts. It sounded like she had ovarian cysts removed from her ovaries, not her actual ovaries removed.

If April had tumors that necessitated both ovaries being removed she would have undergone chemo/radiation too. I have a hard time believing a “cancer survivor” wouldn’t be front and center on her bio if she went through that in her 20’s. Ovarian cancer in your 20’s is deadly serious shit. No way she’d mention “can’t have kids,” or having a total hysterectomy, but not also mention surviving cancer or the reason for such a major procedure in her 20’s. They absolutely do not remove a woman ovaries in her 20’s unless it’s a life or death matter.

This is definitely what happened. And it's why Nick is so mad. Aaron is telling the world "he got out." In reality, April decided to stay with Nick and drugs when Aaron had to go back to his house for weekends with his kids. April staying with Nick pissed Aaron off. Aaron then pretended he was with Kayla so even trade but Kayla didn't want him either and again he's alone every other weekend. He became angry while Nick is convincing Kayla and April that Aaron is paranoid/drugged out and bad for everyone. They kick him out. So now he has no drugs, no friends, no wife and no girlfriend. He starts spewing a tale of heroic escape. This so contradictory to Nick's tale that convinces everyone Aaron needs to go that he can't stand it. Nick is fuming "all 3 of us decided Aaron was the paranoid/coke problem. Who are you going believe: 3 of us or Aaron?" Nick is fuming that he can't put Kayla, April and himself on a stream telling the world they kicked him out. In pure Nick fashion, after convincing the girls that Aaron needs to go by twisting everything he did, he'll passive/agressive it with "I didn't kick Aaron out, the group decided he was crazy and paranoid and couldn't stay over. April divorced him all on her own."
