Yep. There are three main reasons for this, but the most significant is that the anal cavity is more delicate than its vaginal counterpart. Other, less important reasons include (1) the butt has many of the cells HIV targets, and (2) butt slime is inferior to cunt slime.
You may be wondering what I meant when I said that the vagina is less fragile than the anal cavity, or why that matters. What I mean is that the anal wall is made up of a single layer of skin cells. These specific skin cells are called “columnar epithelium”, which basically means they’re tall and skinny. Vaginal epithelium is squamous - so shaped like a scale - and consists of several layers of skin cells.

A single layer of lanky skin cells is easily scratched open. By contrast, layers of scale-like cells are not. Such cells can handle mechanical stress without tearing. If they do tear, there’s another layer of cells beneath them. These underlying cells run interference and keep the virus at bay. You might be wondering how someone could be oblivious to an open wound on their crotch, and the answer is that they’re very small cuts. So tiny, in fact, that they’re best conceptualized as painless “micro-tears” and not something that produces visible quantities of blood. These micro-tears can be extremely subtle, whereas tears to the vaginal cavity are pretty goddamn obvious.
As for the other, less significant reasons…. The skin of the anal cavity is rich in CD4+ T-cells, dendritic cells, and microphages. It doesn’t really matter what these are - what matters is that they’re the kind of cells HIV targets. Even if the vagina and anus had equal chances of micro-tears, the anal cavity would still have more of what HIV needs to establish itself.
Finally, vaginal mucous is pretty useful. Part of this stems from the fact that the vagina has a whole ecology of microscopic life, like the bacterium Lactobacilli. Their metabolic processes produce lactic acid, which lowers the vagina’s pH. This acidic environment protects the woman’s body by burning up potential threats. Even if a woman’s pH is off, the vagina produces a lot of slime and the vulva’s various glands help. They lubricate sex, which prevents skin tears. Mucous also swallows up semen and introduced debris like a lava flow sucks up trees. As the woman orgasms and goes about her day, the vagina moves out all this gunk like a conveyor belt. This keeps any infected fluids from pooling on top of the skin.
By contrast, your butt shouldn’t have much life, and that life should not be creating an acidic environment. This lack of acidic environment means your body is less protected. Your butt doesn’t make much slime, so anal sex can be extremely dry. Unless lubricant is added, this dryness will create friction that wears the skin down and leaves it subject to infection. Finally, this lack of mucous means that fluids introduced to the anal cavity can sit there. This provides ample opportunity for the virus to melt into worn, inflamed skin.
TLDR: Anal sex is extremely dangerous regardless of whether it’s happening in the context of straight or gay sex.
