Well legal size to be a midget is 4'10", so Ralph has an inch or two advantage here, but I'll assume we're talking more like a 3'2" midget. Either way He'd fuck Ralph up.
The midget would just run after Ralph's legs dodging in and out under the gunt like a methed out goblin, and with Ralph's already slow reflexes he wouldn't be able to do a thing about it. He'd try to lumber after the little guy but a midget doesn't get to be a Mexican cartel member by being a bitch, and he's on bath salts. That's not even fair, it would be like a wolf hunting a moose, getting around behind Ralph and biting chunks out of his thighs as Ralph tried to flee in terror until he bled out.
Give Ralph a fight versus a regular non-cartel midget who's sober but pretty mad and the odds might be a little more even.

Reach is a problem, but I remember an AMA wherein a prison guard mentioned that when it came to violence, it was the skinny guys who were the most trouble. Sure, you had these guys who did nothing but work out all day every day and grew into miniature Hulks, but those guys were so heavy and muscle-bound that they had only a few good swings before they were tired. The skinny inmates were weaker, but mean as hell and had lots of stamina for a prolonged brawl.
If it's a cartel member, I'm assuming it's somebody who's been in fights and is used to moving around a lot. Ralph is, well, Ralph. He'll go down on his back and flail around like a flipped turtle, just like Ralph in Lisbon.

The second one. Very small, unarmed, and has no reach but Ralph is sluggish especially when drunk. The bath salts work their magic and allow the midgit to cling to Ralph and scratch and bite him into submission.
