Hisashi Ouchi probably holds the title for the worst physical pain ever experienced. He received an extremely lethal dose of radiating as he loses his skin and anything else he would've been able to regeneration had it not been for that radiation. He was kept alive for 83 days with most of those days being devoid of skin and flesh and without a doubt the extreme pain that comes with it.

As for the most torturous life imaginable. Imagine losing your eyes, arms and legs while also being rendered deaf and mute but still being conscious throughout all of this and with that being stripped of your right to die having to wait until old age to finally die. Johnny Got His Gun is pure nightmare fuel.

Solution
The end of The Mist, where the guy mercy kills his wife and children and is about to off himself only to see the mist clear and people coming to save them. That always stuck with me.

It's unfortunate being name Ouchi, fate doesn't have good things in store for you.

Once read a story about a guy trapped in a steam tunnel. Apparently it's as excruciating as being burned with one difference- with fire the nerves are destroyed after a short while numbing the pain as a small mercy for unfortunate people who die that way- but with steam similar pain is felt but the nerves aren't destroyed. The pain keeps being felt until it death.
 
Prolonged natural childbirth without pain relief was the worst agony I've ever experienced. It was worse than any bone I've ever broken.

The worst pain ever would probably be something like being slowly cooked alive in a brazen bull or oven, or being slowly crushed to death.
