It turned up in the DLC for the first Outlast; you run into a lunatic who's capturing other inmates in the asylum and trying to make them into a woman he can marry by slicing off their junk and stuffing their chests, which goes about as well as you'd expect. You can find the results of his handiwork at one point, and it's legitimately pretty spooky and fucked up. Still wouldn't give it a whole trope, though.

So, you know how an lot of cartoons tends to outsource the animation department to an different company? An scene where an character slips on some loose papers and one of them just happens to be from the Qur'an; which unsurprisingly pissed off the Muzzies into reporting the show's YT channel and subsequently forcing the offending parties to edit an old episode that was 8 years old, from then. Now, at this point, you're probably wondering why Muzzies acting retarded over an children's show has anything to do with TVT. So the reality is that the preceding bit of trivia was asking for someone to raid that YT channel, in their mind.

Kory putting an end to renaming offensively titled pages because within a decade the current term will magically become "offensive" and has to replaced with something new because people can't get rid of their emotional baggage

I'm pretty sure that they have an page or two for "lazy bureaucrat/politician resorts to killing off people just to stay in power" but that's also something that probably didn't start with Rome. But even then, it's not necessarily an case of being completely incompetent, either.
