Not Alzheimers, but there are some rare disorders that work like this. Inability to form new memories, wake up every morning in nursing home wondering where you are and how'd you go from 30 to 80 overnight.

Unrealistic in this case though because it would place his transitioning somewhere in 1970s - 80s. Trannies were rarer than unicorns back then, and the statistical chance of one of them developing something described above...

I edited this drawing to explain why there's no simple answer for philosophical arguments about the trans movement. It's bland as heck and no punchline, but I guess if there were a punchline we'd know what to do.
