As funny as the idea is, this doesn't sound super realistic to me. We have a "body map", but it's based on our actual bodies, not on some sort of Platonic ideal of what our bodies should be stored in our minds, so why would he somehow be able to accept his 80 year old body, but not the aspects of it he developed in his 30s? Do people who received amputations in their 30s and then develop Alzheimer's later in life freak out about their limbs being removed? Maybe they do, but when I looked into it I couldn't find anything about it (though I did find stuff about the ethics of performing amputations on people who already have Alzheimer's).

Anecdotally: no, they don't, but I've also dealt with people who had amputations five or ten years before their dementia diagnoses who were variable about whether they remembered it. It'd make sense if there's a cutoff for remembering body changes.

I would think that the real problems with Alzheimer's and post-op transsexuals would be when it's the AGP late midlife crisis, the coomer grandpas who speedrun a vagina in a year or so on the strength of the "I've always known; now let me die a woman" argument.

The hand that used to sign a lease on a red Corvette now signs for a stinkditch, as impulse control fades but the checkbook hasn't been impounded yet. Lots of grandpas go in for an implanted erectile device around the same time, with the same failing reason and restraint. Once the Reader's Digest set learns about penile-preserving vaginoplasties, maybe they'll start signing up for a twofer.
