Toxic BL is for relationship drama. "Whump" is the new-ish term for male-on-male physical abuse that inexplicably gets clits hard. I think women like it because the suffering is more "equal". Things (like mainstream porn) where men are violent towards women (dark romance books are different because the violence is "controlled" and "consensual") make women uncomfortable because it's too real and personal. Is that a newer development?
"Whump" as a term has been around since the early 2000s, and it's always had a big overlap with "hurt/comfort." If whump in a fanfic isn't a lead-in to caregiving, you can expect it to be tagged "hurt no comfort" so readers know what they're getting into; the opposite is assumed.
Hurt/comfort is a lot of writers' first stories, but I think it's simpler than that. Much as two male characters falling in love can be enjoyed at a safer remove, hurt/comfort can and is often written nonsexually, which hits a much younger demographic (both sides).
The other reason is that you see it in canon. Nobody ever filmed a scene where Kirk and Spock make out, but as an example, dang if most of Operation - Annihilate! isn't Kirk checking in on Spock, telling him to rest, worrying if Spock's going to be OK after that flying pancake attached to his spine.

It's a different sort of misogyny now: porn has got so extreme and so widespread that kids see horrific rape and torture porn before they even develop a sex drive of their own. We know porn influences lots of boys to troon out, it's probably the same shit for pooners.
