Variable. You'd go from one street to the next and suddenly be hit with an overwhelming stench of raw sewage. There was a general bad smell that I sort of got used to.
I ate at any seemingly reputable restaurants I could find (which was not an easy task), and was in a pretty good hotel for the majority (but only ate breakfast there). Ate absolutely no street food and drank 0 water that wasn't bottled. I didn't experience any significant stomach upset - I'm sure I would've if I hadn't been extremely careful. Food in restaurants very obviously used low quality meat, curries often came with chicken legs instead of chunks of breast/thigh like I'm used to, said chicken legs were very very small and gamey.
Consider yourself lucky. We know a Swiss colleague who went for a visit and came back with a lingering gastrointestinal illness that still affects him today. The doctors in Switzerland are perplexed. He would probably have to go back to India and see their top tropical medicine people to figure out what's going on because there's no way a western European doctor has the experience to deal with that.
