Immigrant visas require a medical (where the CDC could incorporate a physiognomy check quite easily), but for some reason H-1B and many other non-immigrant visas do not. Since apparently you can't spread diseases to the US if you're only there for 3-6 years.

If Trump was actually a fraction as based as some claim, he could require medicals for H-1Bs and that would completely gum up the system since there's an extremely limited number of physicians who are approved to do these medicals and they are expensive. Mine cost the equivalent of $500 USD for a urine test, 1 hour consultation, and analysis of an x-ray (which I had to pay separately for).
