The KSA announces the immediate disbandment of the Kerbal Space Program after it's inagural flight killed lead astronaut Jeb Kerman and dozens of staff at the space center, after someone accidentially set one of the three stag 1 solid boosters to half strength and the rocket overturned. KSP

And perhaps my favorite. The American people's history are lost to the annuls of time, when while searching for a homeland, they were all eaten by a rampaging pack of bears. CIV V
 
Dr Mario - Fake doctor faces charges. Victim's families demand death sentence to the Plumber of Death.
 
Knights of the Old Republic. The amnesiac dark lord Revan, who is unwittingly serving the Republic at the time, is gunned down by essentially a storm trooper literally 5 min into his first assignment as a republic soldier. The entire galaxy falls to Darth Malak as a result.

No word on if this is actually not so bad for the galaxy like when Palpatine pulled the "take over the galaxy" thing though.

Dark Souls 2: The Bearer of the Curse could eventually become a powerful being on par with the Chosen Undead after fighting foes like the Burnt Ivory King, the Fume Knight, and Nashandra. Before he can reach the three old fire keepers, he decided to fight the weird monsters in the grass on to get ganged. Though he could come back, the bridge is somehow destroyed, thus keeping him trapped with the small creatures and a nearby ogre.

I'm going to be Autistic and point out you can walk past the Ogre to get to the hut.

Dark Souls 3: The Ashen One reaches Firelink Shrine only to be immediately cut in half by a Katana Hobo.

Then the player destroys their console in frustration, leaving the world to be unchanged forever.

In that case: Even if the Chosen Undead makes it past the ogre, the undead lacks enough brains to go to the forest and instead heads for Heide's Tower of Flame. The knights within that area cause the undead to lose all healing items in botched attempts at conquering them.
 
Dark Souls 1: An undead who has not lost his mind to hollowing in the Undead Asylum is given a new chance at freedom only to be killed by a large boulder rolling down a staircase while making his way out.

Dark Souls 2: An undead makes it to Majula only to be mauled to death by boar piglets bursting through a closed door while he talks to a cat.

Kerbal Space Program: The space program shuts down after running a deficit trying to rescue two Kerbals stuck in orbit because they ran out of fuel on their return trip from the Mun.
 
Super Mario Brothers: Mario must rescue the princess, but the first Goomba he encounters is too big to jump over and he dies immediately.
 
Elder Scrolls IV: Oblivion

The Hero of Kvatch emerges from the sewers of the Imperial City, crosses the lake, and arrives at a strange Ayleid ruin.

Only to get shot in the shoulder by a bandit's arrow and die in the ensuing conflict, thus dooming the entire world to being subject to the will of Mehrunes Dagon and causing the downfall of civilization as we know it. The Amulet of Kings is discarded in the middle of a forest because no one could ever put it on. Martin dies in Kvatch to a Clannfear. The Mages Guild is consumed by Mannimarco. The Blackwood Company completely destroys the Fighter's Guild. The Thieves Guild is eventually disbanded as the Gray Fox is unmasked to be.. someone. The Dark Brotherhood is completely uprooted by internal destruction.

The worst thing about all this?

The Adoring Fan never went on the adventure of a lifetime.
 
Hotline Miami: Jacket walks into the hallway, a dog runs out of nowhere at the speed of sound and mauls him.
 
Mass Effect: Sovereign spots the Normandy as it's coming to drop Shepard, Alenko and Jenkins on Eden Prime and destroys it utterly. The Galaxy falls to the Reapers with minimal resistance, and the harvest continues as per usual.
 
Amnesia: The Dark Descent

Daniel is killed by the first monster he encounters. Alexander, completely unaware of the revenge plot, returns to his native dimension. That one guy is still chained to the wall, the castle is still crawling with abominations, and basically nothing changes.
 
Fallout 3: The Lone Wanderer is beaten and shot to death by the Vault 101 guards on the Overseer's command; any other riots are just as swiftly put down until the Vault returns to its uneasy state of peace and stagnation. James is trapped in the hellish Tranquility Lane simulation forever; never knowing what became of his child. Eventually the Enclave shows up and takes control of Project Purity so as to poison the water supply. Everyone in the Capital Wasteland not living in a vault or abducted by aliens dies horribly from FEV contamination.

Bioshock Infinite: Booker DeWitt survives the mob of Columbian cops after refusing to participate in the interracial couple stoning; only to plummet to his death from a Skyline by misjudging a jump with his skyhook. The Vox Populi are swiftly put down, and Elizabeth is doomed to being experimented on and conditioned in the Comstock House until she finally fulfills the purpose she was obtained for and rains hellfire down from the sky on New York in 1983.

...I shouldn't play games where the fate of so many people rely on my protagonist not being a fuckup, to be honest.
 
Resident Evil 4: Leon goes fishing and gets eaten by a salamander.
