Mafia is specifically sicilian yes, but I doubt the author is enough informed to not use mafia as an umbrella term for italian criminal organizations.
As for the closest correlation between mafia and black people, well, there's human traffic of workers. Immigrants from middle Africa that work underpaid, with no working or sanitary rights, in borderline slavery conditions. The worst case of this is the luckily dying case of Negri occhi rossi (red eyes niggers) which are immigrants who work and live in orange camps, in very small tents, and are forced to eat mostly oranges to survive, which makes their eyes red.
