What's ironic about this picture, and the symbolism it represents, is that the man with his eyes closed is only holding the reaper at bay, not sending him away. While, more importantly, holding the Caduceus, the symbol of commerce and negotiation, instead of the Rod of Asclepius, the actual symbol of medicine, health, and The Hippocratic Oath.

I don't know what that one idiot is doing, touching the tip like that, but what you're looking at is a safety-lock intramuscular tip (after you give the injection you snap the lock into place, to prevent accidental needle pokes).
