Ralph streamed from his Portuguese hosts' house after being left out of the hospital. His teeth being missing was a talking point and he addresses it.
I don't remember exactly how Ralph debunked it. (Could have been a lie)
Everyone dropped it after that.

And around the Ade Zero arc he did mention going to the dentist several times.

Now this is funny. He went apeshit during portugal 1.0 about how he didn't lose teeth. It looked suspicious because it was the same way he always lies. LOLOL HAHAHAHA THEY THINK RALPHAKING LOST TEEETH LOL HAHA LOL HAHA. He only acts that way when it's a cope, maybe he already lost them to bad hygiene or maybe a Moorish purse salesman really did collect a few trophies. Either way, it's funny.

This is the most important point. I’ve always said he had partials or dentures just because guys like Ralph usually do by 30 or 35. Ralph’s teeth don’t look real for a guy like Ralph, even if they are yellow they aren’t chipped, broken, decayed - because they are fake. He might have partials and veneers. He just doesn’t clean them very often so they get yellow.

It looks like he had lost them, or took them out, after the beat down because he would have to while he healed up and had surgery done. He might have lost teeth during the beat down but then he would just go from partial to full dentures.

I think most people just don’t recognize dentures, veneers, or hair extensions even though they are very common.

I remember it was a revelation to some twenty-something white guys that black girls with long straight hair were wearing wigs. I pointed out that unless black women have a fro, braids, or that flat iron sheet hair then they are wearing a wig or a weave. They were gobsmacked.
 
Until I see him take a few big bites out of a Granny Smith Abuelita Gonzales apple, I'll choose to believe that he's wearing dentures. That reminds me, did we ever get a video of PPP tying his shoes?
