Most churches in coastal states excommunicate people who come out as gay. Even some of the Methodist churches do it, despite the fact that Methodist churches elsewhere welcome them.
Not to mention that in many, many southern states coming out as gay will get you beaten up, if not killed.
I am genuinely curious about where you got this information. Did this happen to someone you know? Because it's completely opposite to everything I've seen myself and heard from others. I've lived in coastal cities and gays who want to just go to mainstream churches where the clergy are super gay friendly (not to the brown people/immigrant churches where that might get you shunned). I have redneck/country relatives and they just don't care if anyone is gay. It sounds like a European's perception of America to me. (Genuinely not trying to be a dick here.)
Can we please stop with this idiotic debate about whether or not it can be hard or dangerous to be gay in the US? I'll repeat what I said before: The great old American melting pot means we are so big, so varied, and so stupid that parts of this country are progressive while others are basically the developing world.
And it can change even in the same state. For instance: being gay in New Orleans? Almost mandatory. Being gay in Baton Rouge? No one gaf unless they're a church lady with a single gay kid they want to set you up with. Being gay in Shreveport? "he just hasn't found the right girl yet." Being gay in Lafayette? Might as well be 1957. And even then it can still vary based on your social set, especially among religious circles.
Or, say Texas. Austin is one of the big three of the gay Meccas of the Deep South along with Nola and Atlanta. But outside of most major urban areas it can be like a horror film. Georgia has Atlanta, Savannah, and Athens. But the smaller towns are much more difficult.
Fuck, I went to school in West Virginia. And while our school was openly welcoming and promoted queer life (Methodist), it was well known that if you were in Certain Counties or took Certain Exits, you kept to yourself as much as possible.
It varies. And anyone who tries to claim that their personal experience with almost anything in the US is the norm is kidding themselves. We are the Nation State equivalent of Frankenstein's Monster. That's partly why so many cults thrive here.

The funny thing is that during Ben and Jessa's courtship/engagement, it was framed as him pursuing his lifelong TV crush. Which was pretty believable at the time. The Duggar girls in general are fairly good looking, but Jessa is exceptionally pretty and has always been pegged as "the hot one." It would make sense that she's the Fundie equivalent of Belle Delphine or Margot Robbie or whoever teen boys are obsessed with these days.
It is really ironic how things ended up with her vs Jill. Everyone in Duggar fan circles thought that it would be her and/or Jinger who broke away and go on to expose the whole thing then live glamorous, more normal lives. And it was also expected that even if she didn't break away, she's probably make the "best" marriage. Now Jessa is stuck with four children (and counting!) In a two bedroom house with a husband who is completely dependent on her father financially and spends his spare time apparently on Grindr instead of looking for a real job while she piles up diapers in her disgusting home and posts totally not fake dialogue from her kids on social media. Ben is supposedly some sort of preacher but clearly his job is, uh, not very time consuming or profitable. He definitely seems a lot less successful than Jinger's husband Jeremy, who is also a pastor but actually works during the day and has a real income.
In contrast Jill has two kids in an appropriately sized home, endorsement deals, a therapist, eats non Carb-age meals, has a nice church community and friends outside of Christian Fundamentalism, dresses however she wants, and has a loving husband with an advanced degree, actual career prospects, an actual backbone, and willingness to acknowledge that she's an abuse survivor. And they're straight up calling Rim Job a practical human trafficker online (apparently Derick runs his social media posts about her family by her before he posts). And her kids are going to make it to fifth grade knowing the alphabet.
But hey, Jessa managed to take Jill's place as favorite daughter. So I'm sure it's all worth it.

Yes, I have a family member who was kicked out of his church just because he came out. They said he could repent (stop being gay) or leave. He left. Many people in our family shun him too. A couple of guys that my son went to school with have been physically assaulted for being gay. As @FernandoPooTragedy said, some places are progressive, some aren't.
I'm not sure, but the only things I can think of are car accident, really bad fall, if they need to be trached (if only so they don't pick at the damn thing), or if one of the parents shook the poor thing like a maraca.
And judging from the lack of bruises, and cuts (at least from what we can see), I unfortunately think it's the latter in this case.
