If she was a size 18, which judging by her body shape I don't think she is. The problem I see for PT is that she just doesn't try to make the rest of herself look better- she just assumes that if she throws money at a cute piece of clothing, it'll make her look so cute that people won't notice the chub or the weird face structure, or the unwashed hair.

Personally I don't even see how she brought herself to throw up; if her symptoms were anything like she says they were, she'd be too busy writhing in pain to even try and the pain probably wasn't in her stomach. I personally have had an episode of heartburn where I was this close to going to the emergency room because I thought it was something more severe. After a few antacids/pain killers/glasses of water, I woke up a few hours later feeling much better. (The lesson there is not to use orange juice as a mixer with tequila. EVER.)
