If that's the case, does the ballot become invalid and removed entirely, or does it just get sent to humans for processing?

It is left in a bucket somewhere in case there is a recount called. Most of the time the invalid ballots wouldn't decide an election, so they're just ignored, but in the event of a tight race the state is required to recount by hand.

Pretty sure Europe has voter registration/voter ID laws just about everywhere, though. Even in the former Warsaw Pact states.
Heck, there's a bunch of third-world shitholes with better measures against election fraud than the United States.

My girlfriend is Moldovan and I recently went to vote with her in her countries' election, their government actually set up polling stations in countries where a large amount of the diaspora moved to (So, Italy, Germany, UK and USA).

Even these guys, whose country is so corrupt they half-ass the election fraud (someone recently found a bunch of half burnt election slips for the opposition in a landfill from the 2016 election) have some pretty stringent rules on voter ID. I wasn't even allowed into the polling station when she went.
