The Dark Spire on the DS. It was published by Atlus in the west so I don't know that it was too obscure at the time, but fair to say it's probably pretty forgotten by now. Fantastic soundtrack considering the limitations of the hardware.
One of it's signature features was an effort at creating an operatic baritone "vocalist" which you can hear in some of these tracks.
Another cool thing they did (corresponding to an in-game toggle) was to convert the entire game to early PC-style visuals and audio, so almost every track in the game also has an 8-bit version.

Va-11 Hall A is a lame anime game virtual novel with pedophilic undertones (one character is an android child who is a prostitute. It's treated as creepy in universe but it's still in the game so make of it what you will) in a universe where YiiK (yes that YiiK) is considered one of the best games of all time.
That being said its soundtrack is primo stuff for vegging out at work when there's nothing going on.

Following on some very excellent taste in Ridge Racer, while not really obscure up to a certain age bracket the Outrun Coast2Coast album is just full of bangers my top three choices when i'm driving and drifting on my home rig.

You already know I have to mention two songs from No More Heroes. In particular the Rank 4 boss theme from NMH 2. For some reason this fight has two different themes depending on the region your copy is from.

The main menu for Ball Breakers (a very obscure PSX game that everyone has forgotten except me) has a pretty slick techno theme. It's very "of its era," but it's still great nonetheless

And the title theme for Spider-Man and the X-Men: Arcade's Revenge is some of Tim Follin's best work imo.
 
Calling anything composed by Uematsu, written by Sakaguchi with art by Toriyama "obscure" may be a stretch, but the game is twenty years old and seems to be forgotten by everyone except my immediate family that played it till the discs wore out.

Stand out tracks for me are Mecha-Robo Corps Charge and State of Emergency. Though I love damn near every song including the ambient tracks.
 
The game itself was very unimpressive (it's an early MegaDrive/Genesis action JRPG but there are seriously several better-looking and better-playing JRPGs on the Sega Master System) however Super Hydlide had background music that was like the most awesome elevator music you've ever heard.

 
This might not be super obscure, but I'll throw it anyways. The only other people I see praising this game are people I don't want to associate with.

The game is extremely brutal and fucked up and as a result almost all the music is extremely strange and that's the part I really appreciate.


Edit: YouTube comments are not their typical trashfire and reflect the music somewhat accurately as well.

Albion is a whoopass-good RPG and I've had the OST on my hard drive for 20 years. Astoundingly enough it's getting a fan remake.


The haunting electronica of KOPS will forever be in my heart because of the 4-way keyboard crowding multiplayer matches we had as kids ;'-}

As will the opening wails of "Dream" from Whacky Wheels

And we can't go without mentioning System Shock 2 !
edit: I'm only considering this one 'obscure' because the thank-you-Lord remake of SS1 stole all the thunder.

Finally an OST that was better than the game. Significantly. I give you "onEscapee".

this isnt obscure but when you say c&c soundtrack most people probably just think of hell march but the entire ost was kino as fuck. this was the music that played in the game installer for fucks sake

Not sure if this counts because the game was never released, but if you like Red Army Choir music, this soundtrack is great:

The game was supposed to be set during the Russian Revolution but IN SPACE, where you were a captain of a ship and could help either the Tsarina or the revolution. The devs made a bunch of artwork and music and stuff, but I think ultimately the Kickstarter failed and the project was unceremoniously abandoned.
