I wouldn't characterize him as "most insane," but Mswati III is a pet favorite of mine. He is the King of Eswatini (AKA Swaziland, it was renamed Eswatini by Mswati himself) and has absolute power. Anyone who criticizes his government is generally killed, he is a profligate, and he currently has 16 wives and 45 children between all of them. The dude spends money on himself and the royal family like nobody's business and most people in the country live in abject poverty. Political parties are verboten and the one TV channel is Eswatini is under his control. Eswatini television programming is very North Korea-esque, but with African flair, complete with a sign language interpreter in the corner. Pro-democracy protests are put down with decisive lethal violence. Mswati discussed physically branding anyone HIV positive and he regularly kidnaps young girls so he can fuck them or turn them into his next wife.

Eswatini is oddly okay with LGBT people for an African country and Mswati mainly kills people who try to threaten his political power and control. Mswati is also very much TAIWAN NUMBAH WAN, visits Taiwan frequently, and is a huge proponent of recognizing Taiwan rather than the People's Republic of China for reasons that aren't quite clear to me beyond Taiwan sending them a lot of foreign aid. Swaziland/Eswatini has been super pro-Taiwan since 1968. When communists launched protests in 2021, the communists claimed he fled to South Africa, pro-Swazi South African groups said he fled to Mozambique, while the government said he never fled at all. Also, look at pictures of him. He looks like a fucking dork.

While he cannot be considered a dictator if we go by the definition of that word, Hong Xiquan is also worth mentioning. He failed his exams multiple times, had nervous breakdowns over them, declared himself to be the brother of Jesus, created a cult, and started the Taiping rebellion, the worst rebellion in Chinese history, which lasted 14 years and resulted in 30 million Chinese dying.
