I am not an expert but I have been researching this as of recent out of my own curiosity. Fuel economy is not an issue in a light duty (3/4 to 1 ton) truck when paired with the right transmission; it has been reported that the 4-53t can get mid to high teens in fuel economy, and a 3-53 into the mid 20s. This probably requires an overdrive and a not too deep rear diff ratio, and if using an automatic transmission, a unit with a locking torque converter would also be desirable. Power figures aren't great, but better than 110 for the 4-53t, later revisions of that motor were rated at 170hp/400 ft lb tq which is low but serviceable if you are not a retard and know how to merge properly onto the interstate. The 3-53 makes 70hp and ~200 ft lb tq, which dips into scary territory, but I think it would be really funny to rip around town in one. People have performed these swaps in the past so it is not entirely out of the question, there are at least a dozen examples on youtube. This swap is stupid in terms of practicality, but I think it would be worth it for the sound and comedy factor.Diesel two stroke is as nuke proof as you could possibly get, but it's also like 6 mpg for like 110 HP, and weighs so much that any retrofit would require using a heavy duty frame.
But you could also get rid of your reverse gear, wire a reverse switch on your starter and drive backwards with all X gears your car has because a NA diesel two stroke can run backwards with no changes.
