Originally it was just a gal who you could pay to help keep you comfortable during childbirth. Rub your back, bring you drinks, basically things husbands are expected to do but if you have a doula you have two people attending to you instead of just one- or sometimes if the father is away in the military or whatever. The word is just Greek for "servant."
