There's a current school of thought that relapse precedes people stopping their medication. People can certainly be 100% compliant and still have episodes, but the side effects are also another reason people cease their medication (it's one thing to tolerate side effects short term, another entirely to endure them indefinitely, especially if their effectiveness is limited).I know and have known people who were bipolar and either relapsed into instability, or usually just decided they weren't taking their medication, sorry the POISON, any more.
The psychotic disorders are a messy area of medicine and we really only have blunt instruments to treat them, although strict social scheduling can certainly help prevent hypo/mania. It's an area which desperately needs more research and especially more treatment options.
