It's probably not clear yet. Brain damage can take months of recovery, and the extent of recovery can vary significantly. He's out of his coma, but nothing more has been heard from him so far, which indicates that he's probably not well enough to speak publicly. I'm almost certain that his social media is being managed by other people at this point.

Aside from that, the art piece was probably intended as some flattering tribute to a queen of intellect. Instead, it's an unintentionally hilarious mish-mash of symbols more reflective of the actual situation from a try-hard faux intellectual the caliber of Kraut.
Or, as @Steamboat_Bill said, it's an elaborate troll.

I'm getting bad feelings about this. I immediately questioned why he would go to a country he's been shit talking for years to seek medical treatment. Now I'm starting to wonder if Mikhaila did this on purpose. Like daddy issues taken to the extreme. "Now you can't tell me what to do, Dad, I will wear that bikini for as long as I want. He's only shit-talked the Soviet Union. I have no idea how he feels about modern capitalist Russia, but he genuinely believes that Tsarist Russia was a paradise compared to the Soviet Union. (As you can see, he gets his knowledge on folktales from Disney films and his knowledge on history from Don Bluth films.)

Putin is a KGB guy. He wants the power and control that Russia lost when the Soviet Union fell. Who knows what type of drugs they have to fuck his higher brain functions up, while still keeping the motor skills to stand upright and drive a remote control car.
I have a wild imagination, and I hope it's not true. I hope he's just taking time to relax and recover, and Mikhaila is just a regular attention whore. But it's Putin. And I've seen many stories about people fucking up loved ones on purpose.
