As someone who actually meets patients like this (rarely, but it does happen) I think you explained the issue much more eloquently than I ever could. It’s a difficult situation, both because you’re instinctively morally troubled by this person’s sexuality and because you’re wanting to prevent him (it’s always a man) from offending. The whole “just hang them by the neck until they’re dead” thing might sound good on the internet, but it’s not very useful when you sit in your office with someone who a) has no criminal record b) clearly is in enormous distress and c) you sort of have an obligation to try to help.
