As far as I'm aware the arm skin ones feel no pleasure, but there's a different surgery called a metoidioplasty that probably just makes normal female orgasms. It's basically where the clitoris, lengthened and atrophied by testosterone, gets sewn into a weird micropenis shape (2 1/2 ish inches in size).
