It's not that unlikely. I recently met someone who has urinary incontinence problems due to a congenital condition and they later confessed to having a diaper fetish. I've also known furries with bladder or kidney issues that send them to the bathroom frequently that also developed ABDL kinks.

It's something about indulging something that causes so much shame, but beyond the usual kind of piss and shit fetishism also includes a sense of relief/freedom from something that causes daily stress and fear.

That said....it definitely doesn't make it any less nasty.
