I think that at the start, this was probably intended to be gayfabe in order to hype the "intervention" stream, but Ralph is so hammered, and his ego is so wounded, that he can't let any minor slight go unchallenged. Milo was obviously trying to cut a funny promo and wring some donos out of his appearance on his buddy's show, but when Ralph fired back, the Old Queen took off his earrings and pulled the razor blade out of his hair.
