I think I read about this clip in particular. The guy broke a rib and punctured a lung.

The big takeaway is that you're not spotting unless you're constantly prepared for the bar to drop. Otherwise you're as useful as a passive spectator. In return, have a good spotter, or a spotter in general, as people can and have died from strangling themselves via barbell.
