I don't know how well known this is, but it's pretty disturbing.
There was a serial killer in USA at the early 20th century, one of the most famous ones- Albert Fish. To quote the Internet on his fetishes: "sadism, masochism, flagellation, exhibitionism, voyeurism, piquerism, cannibalism, coprophagia, urophilia, hematolagnia, pedophilia, necrophilia, and infibulation." He was fried in 1936.
The pic is an x-ray of his pelvis. Guards noticed he had trouble walking and did x-ray to see what was causing it. Turns out he was sticking pins and needles into his pelvis and ass and leaving them there so it will hurt with every move he made, giving him pleasure.
