A good counterpoint to this would be Hitman 3. It has all the content from 1 + 2, but has a smaller file size than either because the team actually optimized. And it has the same size on ps4 as on PC. If IO can do it at their brokest, than these AAA devs have no excuse not to.
It kinda helps that they were using the same engine for years and they built Hitman 3 from the ground up to accommodate all the past content. Working on one type of game for a while leads to all sorts of breakthroughs in optimization hacks and it really wouldn't surprise me if Hitman 1 and 2 had every texture and asset confined to its own 'level' as opposed to a universal texture and asset hub where things can be reused. I can't remember if you could buy levels individually in Hitman 2 but you could in 1 so they had to make sure everything was self contained.

Depends on the game. Hitman has mostly pretty tight levels with a relatively high amount of vertical layering and a restricted entity palette. "Streaming" here refers to asset streaming more in the case of games with lots of detailed assets being swapped in and out, huge levels, open worlds, or simply engines having those options as standard. It's the architecture of the game rather than its size in terms of content.
