Actually, the safety system worked as it should and nobody was injured.

Unless investigation reveals negligence in what caused the coupler failure, then I don't think it's fair to harp on them for this, for once.

But the fact that incidents keep happening is bad for public image regardless. I'd expect some very positive sounding promotions to go out to make themselves look good.

Couplers don't just fail. They're regularly inspected (visual inspection every trip on a decent railway, with a full inspection at the 90-120 days ish mark depending on the maintenance schedule)
Signs of wear, distortion, cracks, failure to properly engage, incorrect height, whatever are all recorded and the rollingstock taken out of service. During manufacture they are inspected for voids in the casting or any dimensional abnormalities and wouldn't be installed if not up to scratch.
If a coupler failed, it was due to negligence.
