The document with the results typically would say "not detected", have your last 4 of your SSN and several codes showing the validity and authenticity of the test itself...
 
The results we get look like this:
Patient name and number
Test: SARS-Coronavirus 2 (2019)
Your value: Negative
Standard value: N/A
Status: Final result
Then a few lines about who requested the test, date and time of test, it specifies both throat and nose samples were obtained.

If a patient needs proof of a negative result, this information goes in a letter with the full details of the patient (not just DOB) which it is signed by a doctor and stamped with the clinic stamp.

Her letter looks homemade.

I am waiting with bated breath for her 2021 resolutions list. Hopefully, we will see again the 1 million steps, the going to university, and the adopting a child and any other totally unachievable goals such as losing weight.

This is the picture you took from my post. That I literally made and printed in 5 minutes using Word. Nothing is photoshopped in it.

So just for shits and giggles I went to the Urgent Care Centers of Kentucky just to see how quickly you can schedule a COVID test. It turns out that the wait time for a COVID test, even a rapid one, is a 5 day-ish wait, unless I am completely stupid and looking at that wrong. She had to have made that appointment on the 15th or before the 15th. Where am I going with this, I have no idea, just FYI.
