I sort of agree because 300 lbs is fixable to return to a normal weight and look normal. At 500 lbs, it's a lost cause, even if you do somehow lose hundreds of pounds you'll look like a deformed monster.

There are two issues. The first one is that to lose weight from 500lbs to 170lbs would require massive calories reduction (~1,500 calories) for a long time, likely years. No one could sustain this amount of calorie reduction for so long unless forced to do so. Clinical evidence has shown that the probability of doing it for someone of her size is near zero. She is already failing to lose weight to get under 500lbs. Bariatric surgery is the only option for her.
The second issue is that her skin has been stretched so much for so long that any significant weight loss would lead to a lot of excess skin requiring surgeries for removal. All of these would be paid by herself of course.
 
I don't think she really comprehends the discomfort that having a lot of loose excess skin provides, and for her the worst of it would be in her gunt and laygs. It's still a heavy hanging weight and it will still chafe and hinder movement. The ones on MSHPL who actually lose the weight are usually begging Dr. Now for skin removal because they are so uncomfortable. And then she will be scarred all over.

Not only that, but ALR was morbidly obese her entire life, even as a fucking toddler. I don't know if this is true but it sounds logical to think that since her body was never in a state of normal weight, it would look even more deformed if she somehow gets there
 
Wouldn't the damage done to her heart+other organs be a lost cause anyway? And her eyes and stuff too. Even if she snapped back, got to 150~ elbees got all her skin surgeries and looked great her heart has been under so much stress she'd die early anyway.
 
Safe to say being morbidly obese for this long has shortened her life span. And I can't help wondering what her skeletal structure looks like, compared to someone in a more normal weight range.

That brings me back to MSHPL. One of the women (Brittany, I think?) who actually lost an impressive amount of weight talked about how she still walked stooped over because she'd permanently damaged her skeleton from all the years of obesity.

If I recall correctly from M6HPL, patients still have vastly improved outcomes even if they've been obese all their lives. It probably helps that she's young, too.
