This image explains how it happens. It's been discussed in this thread probably thousands of pages back by now. Basically Amber is sitting on the couch most of the time. She's also eating most of the time, so she is actively gaining weight while sitting. In that seated position, her ass fat is squished and fanned out in a bulge behind her (see top right of the image you posted) and the added fat she accumulates while gaining weight builds into the "shelf." It's essentially a couch-molded fat pad.

I imagine it's quite similar to those manchild boyfriends that a lot of people have. A lot of the time, it's easier to cut them off and ignore them instead of constantly explaining why it's important not to shit your pants and why your teeth need to be brushed every day, multiple times a day. I don't think she's stupid; she knows what normal people do hygiene-wise, it's just weaponised incompetence at this point.
