I think the quality of medicine in Mexico or other countries has been demonized a bit too much. You can have quality surgery outside the U.S. Someone I know was offered by his insurance company to have a hip replacement surgery done in India instead of the U.S. (Nevada). The company covered the cost of the flights, hotels, and incidentals for him and his wife, with no copay and no deductible.
As for Amber, I think a respectable establishment and surgeon would evaluate if she is a good candidate, and do the surgery if she is. My issue is that Amber is reticent to follow medical instructions and would undo the work of the surgeon.
